The Damage Claim Form is used to file a claim for vehicle damage incurred during the course of your vehicle inspection as a result of actions taken by the Inspector. If possible, the claim should be filed while you are still at the Inspection Facility; but must be fully documented and verifiable. The Excessive Wait Time Form is used to notify inspection facilities of unacceptably long delays while waiting for the inspection system or inspector to be available to start your vehicle inspection. If possible, this notification should be filed while you are still at the Inspection Facility; however, your feedback will be correlated to your vehicle information to review the circumstances leading to your delay. The Customer Comment Form may be used to provide general feedback to the inspection facility. Feel free to submit any comments, positive or otherwise. While we may not provide direct acknowledgement, your comments remain on record and are used for overall performance evaluation at each inspection facility.
